Summary

The Care Concierge plays a vital role in fulfilling Curana Health’s mission to improve the health, happiness, and dignity of our senior living residents. This highly interactive, patient-facing role combines administrative, clinical support, and telehealth facilitation duties to ensure seamless care delivery and an exceptional patient experience in senior living and long-term care communities.

Essential Duties & Responsibilities

Patient Engagement & Coordination

  • Serve as the primary point of contact for patients, families, and facility staff, building strong, trusting relationships.

  • Coordinate care between providers, specialists, therapy, home health, and other ancillary services to ensure continuity of care.

  • Conduct regular outreach to residents and families to assess needs and coordinate services in alignment with providers’ care plans.

  • Support front desk duties including appointment scheduling, patient intake, and general administrative support.

Telehealth Facilitation

  • Set up and operate secure, reliable telehealth technology to support virtual clinical appointments.

  • Greet patients, facilitate introductions, and assist providers during remote examinations following clinician instructions.

  • Troubleshoot minor technical issues related to telehealth equipment and software.

  • Maintain strict compliance with HIPAA and patient privacy standards during all telehealth encounters.

Facility & Community Engagement

  • Act as a liaison between Curana Health and facility stakeholders (nursing, sales, wellness, and leadership).

  • Coordinate and attend facility events, educational sessions, and Curana Health community activities.

  • Maintain open communication with facility staff to identify and relay community needs to the care team.

Clinic Operations Support

  • Assist with clinic setup and design, ensuring clinics meet Curana Health specifications.

  • Manage clinic supplies and collateral to ensure readiness for daily operations.

  • Support clinical functions such as taking vital signs, rooming patients, and assisting with visit preparation (training provided).

  • Serve as backup to Medical Assistant within the on-site Curana Health Clinic as needed.

Qualifications

  • High School Diploma or equivalent.
  • Strong interpersonal, communication, and customer service skills.
  • Organized, proactive, and able to manage multiple priorities in a fast-paced environment.
  • Comfortable using and troubleshooting basic technology, including video conferencing tools.
  • Compassionate, respectful, and committed to providing excellent care to seniors.

Preferred:

  • Registered or Certified Medical Assistant (MA) or Certified Nursing Assistant (CNA).

  • Experience in senior living, long-term care, or healthcare settings.

  • Previous concierge, hospitality, or telehealth experience.

  • Proficiency with Microsoft Office.

  • Familiarity with HIPAA and patient privacy standards.
