Summary

As a key member of our clinical team, the LPN – Care Concierge plays a vital role in delivering compassionate, patient-centered care. This position blends traditional nursing responsibilities with concierge-style service to enhance the patient experience across our care settings. 

Essential Duties & Responsibilities

  • Patient Assessment & Monitoring 

  • Conduct initial and ongoing assessments of patients’ physical and emotional health. 

  • Document vital signs, symptoms, and changes in condition. 

  • Collaborate with providers to develop and update care plans. 

  • Clinical Procedures 

  • Administer injections, immunizations, and medications as prescribed. 

  • Perform wound care, dressing changes, and monitor healing progress. 

  • Assist with diagnostic tests and specimen collection. 

  • Care Coordination 

  • Serve as a liaison between patients, families, and the care team. 

  • Support transitions of care, including discharge planning and follow-ups. 

  • Ensure timely communication of patient needs and updates. 

  • Patient Engagement 

  • Provide education on medications, treatments, and health maintenance. 

  • Promote a welcoming and supportive environment for patients and families. 

  • Address patient concerns with empathy and professionalism. 

  • Administrative Support 

  • Maintain accurate and timely documentation in the electronic health record (EHR). 

  • Assist with scheduling, referrals, and coordination of services. 

  • Uphold compliance with clinical protocols and regulatory standards. 

Qualifications

  • Active LPN license in the state of practice. 

  • Minimum 1–2 years of clinical experience, preferably in a primary care or geriatric setting. 

  • Proficiency in patient assessment, injections, and wound care. 

  • Strong interpersonal and communication skills. 

  • Ability to work independently and as part of a multidisciplinary team.
