As a key member of our clinical team, the LPN – Care Concierge plays a vital role in delivering compassionate, patient-centered care. This position blends traditional nursing responsibilities with concierge-style service to enhance the patient experience across our care settings.
Patient Assessment & Monitoring
Conduct initial and ongoing assessments of patients’ physical and emotional health.
Document vital signs, symptoms, and changes in condition.
Collaborate with providers to develop and update care plans.
Clinical Procedures
Administer injections, immunizations, and medications as prescribed.
Perform wound care, dressing changes, and monitor healing progress.
Assist with diagnostic tests and specimen collection.
Care Coordination
Serve as a liaison between patients, families, and the care team.
Support transitions of care, including discharge planning and follow-ups.
Ensure timely communication of patient needs and updates.
Patient Engagement
Provide education on medications, treatments, and health maintenance.
Promote a welcoming and supportive environment for patients and families.
Address patient concerns with empathy and professionalism.
Administrative Support
Maintain accurate and timely documentation in the electronic health record (EHR).
Assist with scheduling, referrals, and coordination of services.
Uphold compliance with clinical protocols and regulatory standards.
Active LPN license in the state of practice.
Minimum 1–2 years of clinical experience, preferably in a primary care or geriatric setting.
Proficiency in patient assessment, injections, and wound care.
Strong interpersonal and communication skills.
Ability to work independently and as part of a multidisciplinary team.
