We are seeking a compassionate Nurse Practitioner or Physician Assistant to provide primary care services in assisted living, independent living, and memory care communities. This role is ideal for clinicians looking for flexibility, meaningful work with older adults, and the ability to earn incremental income without a full-time commitment. Most on-site work is completed before lunch, with flexible charting time.
Round on residents and provide primary care, chronic disease management, and preventive services
Perform assessments, develop treatment plans, and address emerging clinical concerns
Coordinate care with facility staff, families, and interdisciplinary teams
Educate residents and caregivers on wellness and chronic condition management
Document accurately and efficiently in the EHR in compliance with regulatory standards
Support quality initiatives focused on patient outcomes and reduced hospitalizations
Practice in accordance with state licensure requirements and organizational policies
On-site rounding: 1–2 days/week (up to 3 max), primarily mornings
Charting: Flexible; remote charting permitted per policy
Guaranteed pay tied to committed rounding days
Incremental day-rate for additional rounding days by mutual agreement
wRVU compensation for patients seen

Not a pure productivity model; includes guaranteed base for consistency

This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required.
