The Behavioral Health Care Coordinator supports Curana Health’s behavioral health programs by providing proactive, non-clinical care coordination for patients in long-term care, senior living, and post-acute settings.
This role is responsible for managing a defined patient caseload, ensuring regular patient engagement, coordinating care across interdisciplinary teams, and supporting providers in delivering high-quality behavioral health services. The Care Manager plays a key role in identifying changes in patient condition, reinforcing treatment plans, and facilitating communication between patients, families, and care teams.
This is a remote, high-volume, metrics-driven role requiring strong organization, communication, and the ability to work independently while adhering to established workflows and documentation standards.
